Patients with decompensated hepatic cirrhosis may present systemic alterations and dysfunction of multiple organs. Ascites, portal hypertension, esophageal varices, encephalopathy are common complications arising from cirrhosis. The aim this paper is to report a case patient liver the transjugular intrahepatic portosystemic shunt. Male, elderly, ex-alcoholic, diagnosed cirrhosis, ascites, varices. He underwent shunt due hypertension returned hospital after 24 hours agitation mental confusion. had bowel movement stop, neurological worsening, loss renal function, hydrothorax, encephalopathy, hypernatremia, hypocalcemia, hypophosphatemia. new procedure occlude shunt, showing improvement status ascites. However, continued decompensation hydro-electrolytic disorders. evolved worsening ventilatory pattern, fatal outcome. Esophageal varices can be corrected such as occur. Therefore, there need for reintervention or reduce its caliber. Thus, patients advanced age potential risks benefits should carefully evaluated risk death.
